REGISTRATION INFORMATION REQUIRED & WAIVER:

First Name, Last Name, email address, address, birth date, T-Shirt size, Team Name, Survivor, Caregiver.

All participating plungers must sign a Purple Plunge waiver. Please print, sign, and bring the following Purple Plunge waiver to the Purple Plunge event on March 2, 2024.

JUST FOR FUN:

We invite all Plungers to wear costumes. An award for the best costume will be given at the post plunge party.

$30 Per Registrant (T-Shirt provided with each registration)
