REGISTRATION INFORMATION REQUIRED & WAIVER:

First Name, Last Name, email address, address, birth date, T-Shirt size, Team Name, Survivor, Caregiver.

All participating plungers must sign a Purple Plunge waiver. Please print, sign, and bring the following Purple Plunge waiver to the Purple Plunge event on March 5, 2022.

We invite all Plungers to wear costumes. An award for the best costume will be given at the post plunge party.
